  • Owner

    Eric is a U.S. Navy veteran who enlisted and served as a hospital corpsman. He currently serves as a 1st Lieutenant in the Texas Army National Guard Reserves. A certified physician assistant, he earned his Master of Physician Assistant Studies (MPAS) from South College in Knoxville, TN. Eric also provides medical care to the North Texas community at the Collin County Detention Center in McKinney, TX, with additional experience in neurosurgery and urgent care. A devoted father and husband, he enjoys spending time with family and playing soccer.

    Sheryl is a dedicated nurse practitioner practicing at Medical City Children’s Hospital in Dallas, TX. She earned her Advanced Practice, Master’s degree from the University of South Alabama. A devoted mother and wife, she cherishes quality time with her family and friends.

Our clinic guarantees no extraneous testing, no unnecessary services, and no delays. Every minute of the exam is laser-focused on securing the evidence needed for the highest lawful rating.

    • Full review of the veteran’s VBMS (Veterans Benefits Management System), service treatment records (STRs), private medical records, and the specific DBQ(s) (Disability Benefits Questionnaire(s) ordered. Identification of all rateable elements (range of motion, functional loss, pain on motion, flare ups, DeLuca criteria, neurological deficits, scars, etc.).

    • In-person interview that includes detailed military, occupational, and recreational history. Onset, progression, frequency, severity, and functional impact of each claimed condition. Lay statements and buddy statements integrated into the medical opinion when applicable.

    • Conduct targeted, evidence-based physical assessments limited exclusively to the DBQ-ordered condition(s). No broad “head-to-toe” exams—every test, measurement, and maneuver is tied directly to VA rating criteria (38 CFR § 4.40–4.71a, M21-1, etc.).

    • Referrals for various diagnostic testing such as imaging (X-ray, MRI, CT), lab studies (A1c, TSH, CBC, etc.), etc will be ordered if required by the DBQ.

    • Our veteran-owned clinic completes 100% of DBQs electronically. DBQ is the official VA template (VA Form series 21-0960) that replaces the old narrative C&P (Compensation & Pension) exam report. It is a structured, checkbox + narrative hybrid designed to capture every ratable element under 38 CFR Part 4 (VA Schedule for Rating Disabilities).

    • The most critical part of every VA DBQ. It is a legally binding, evidence-based statement written by a VA-credentialed, board-certified clinician that directly determines whether a veteran receives service-connected disability compensation.

    • 100% peer review by second VA-credentialed examiner and compliance scan. Every DBQ undergoes a multi-layer, VA-compliant QA process before upload to CAPRI (Compensation and Pension Record Interchange)/VBMS. This ensures 100% acceptance by VA raters and zero supplemental requests.

Your Questions, Answered

What is the process after I submit my claim?

After you file your claim (VA Form 21-526EZ) online at VA.gov, the VA reviews the claim then orders DBQ(s) in VBMS. The contractor (Loyal Source) then calls/texts you to schedule the appointment.


Can I choose Flagship Health & Wellness to complete my service-connected disability claims?

Yes. You can pick our veteran-owned clinic under the Mission Act. Just tell VA and Loyal Source: “Please schedule my DBQ(s) at Flagship Health and Wellness located in McKinney, TX.”


What if I miss my appointment?

Call Loyal Source within 24 hrs at 833-832-7077. They can reschedule within the same week. Miss twice? VA may deny claim until you show.


Can I bring my spouse or buddy statement?

Absolutely. You can also bring any medical documents you have on hand that can help your disability claim. We scan & upload it, then it becomes official evidence in VBMS.


When do I get to see my DBQ(s)?

Expect to see DBQ evidence in your VA's Veterans Benefits Management System (VBMS) 7–14 days post-exam, with full content access in 2–4 weeks.
